Initial Assessment $275.00
Subsequent Individual/Couples or Family Sessions $205.00
Payment accepted includes: cash, checks, visa or insurance.
If you desire to use your insurance to pay for the services I provide, please check with your insurance company to get details about your coverage. It is important that you understand the benefits and limitations of your coverage as you are ultimately responsible to pay for the services I provide. I am on several insurance panels as an in-network provider and for others I accept out of network benefits. My billing service will assist you once coverage has been confirmed.
Forms
All new clients will be asked to complete forms for the initial appointment. These include the Consent Form, and the Adult
or Child/Teen Form. If you would like me to coordinate care with another provider, please complete the Release of Information Form. During COVID there is an additional Telehealth Release Form to be completed.
